What is a non-conformance (NCR)?
A non-conformance is simply the non-fulfilment of a requirement: something that did not meet the standard, a procedure, a customer specification or the law. A non-conformance report (NCR) is how you record it and drive it to a real close-out.

Auditors don't just want to see that you found a problem, they want to see that you fixed the cause and proved the fix worked. That is the difference between a correction and a corrective action.
Correction vs corrective action
A correction fixes the immediate problem. A corrective action removes the root cause so it cannot happen again. ISO expects both, where warranted.
What CAPA means
CAPA stands for corrective (and, historically, preventive) action. ISO 9001:2015 folded prevention into risk-based thinking, but the term CAPA is still widely used.
Root cause, then action
A credible non-conformance evidences why it happened, then assigns corrective actions with owners and due dates, not just a quick patch.
Effectiveness review closes it
Nothing should close on paper alone. The non-conformance closes only once the action is verified as effective, with the evidence retained.

1 · Raised
A finding, complaint, incident or defect is logged against a requirement, from an internal audit, the floor or a customer. The clock and the evidence trail start here.
2 · Contained
An immediate correction stops the bleeding: quarantine the stock, redo the job, make the area safe. It treats the symptom, not yet the cause.
3 · Root cause
Investigate why it really happened (5 Whys, fishbone, the evidence) so you fix the cause, not just the symptom. This is what auditors probe.
4 · Corrective action
Actions that remove the cause are assigned with owners and due dates, then tracked to completion. A patch is not a corrective action.
5 · Effectiveness review
Before it can close, someone verifies the fix actually worked and the problem has not returned. This is the gate, the step weak systems skip.
6 · Closed
Only now does it close: root cause understood, action proven effective, evidence retained. When the auditor asks, the whole story is one click away.
